DEVELOPMENT OF HEALTH DISPARITY COLLABORATIVES:
CANCER AND PREVENTION |
Goal:
Develop and test a pilot collaborative in cancer
and in prevention during fiscal year 2001 for
implementation in summer 2002. A 12-month development
and prototype phase for each topic will begin
July 2001.
Performance Expectations:
PCA and CN:
-
dentify and recruit 1-2 health center teams
and senior leader to participate in the prototype
phase for cancer and prevention collaboratives.

Support travel and per diem for PCA clinical
coordinators as appropriate, Information System
(IS) specialists, TOT coordinators, and cluster
collaborative coordinators and directors to
the three prototype learning sessions.
In support of this National Goal, the
BPHC plans to:

In collaboration with IHI, support the design
of the new collaboratives, including updated
registry software.

Coordinate development with the CDC National
Immunization Program.
