What is the Internet Adult Disability and Work History Report for Professionals, Representatives, and Organizations (i3368PRO)?

It’s a time-saving way for you to give us medical records and other information about your client’s medical condition, health care providers, medications, education and job skills so we can make the correct decision about your client’s adult disability claim.

Why complete the i3368PRO?

  • You can complete the online report at your own pace in the privacy of your office or the location you choose
  • You save time and money because you don’t need to make trips to a Social Security office
  • Filling out the Online Disability Report instead of preparing paperwork costs the American taxpayer less to process.
 

Who can use the i3368PRO?

You can, if you:

  • Are an attorney or non-attorney representative
  • Represent a government agency, social agency, insurance company, health services agency, hospital, nursing care facility, homeless shelter, or for-profit organization that assists applicants filing for disability
  • Have access to a computer and printer

AND

Your client:

  • Lives in the United States, its territories or possessions
  • Is between age 18 and full retirement age
  • Has not been denied for disability benefits within the past 60 days.
 

How secure is the i3368PRO?

We go to great lengths to protect personal information. We use the most secure technology on the Internet to keep your client’s information private and secure. 

 

What should I have ready before I start?

You should have on hand the following information about your client:

  • Social Security number
  • Address and telephone number
  • Name, address and phone number of someone else who knows about your client’s conditions
  • A description of your client’s disabling conditions and when they began
  • Names and addresses of medical providers, such as doctors, hospitals, etc.
  • The name, date and place of each medical test taken
  • The names of all the prescription and non-prescription medicines your client takes
  • A list of your client’s jobs for the last 15 years before he or she became unable to work because of the disabling condition, and the dates your client worked in those jobs and
  • Information on your client’s education and training.
 

What if I don’t have all the information?

  • Give us as much information as you have
  • Give us enough information to contact doctors and hospitals to get your client’s medical records
  • If you do not have exact dates, try to give us approximate dates; for example, we will accept June 2001, 6/01, summer 2001, and similar estimates
  • Do not delay completing the report just because your client does not have all the information we need.
 

Must I complete the whole report in one sitting?

No. You do not have to do the i3368PRO all at once:

  • After you fill in general information such as name and address, the disabling condition, and the date the disability started, you’ll get a Re-entry Number
  • Print the page that has the Re-entry Number or write it down, along with the web address shown on the page
  • After that, you can stop any time and use the Re-entry Number finish the report later.

You can’t return to a report you started without the Re-entry number. To protect privacy, we don’t record Re-entry numbers. If you lose the Re-entry number, we will not be able to tell you what it was.

If you lose the Re-entry number and still want to complete the report via Internet, you will have to start a new report from the beginning.

 

How do I move around in the i3368PRO?

“Continue” button
Use the “Continue” button at the bottom of the page to move forward page by page

“Previous” button
Use the “Previous” button at the bottom of the page to move backward page by page. Do NOT use your browser’s “Back” button on your browser to move backward

Tabs
Use the tabs at the top of the page to move from section to section. The tabs take you to the first page of a section. If the tabs aren’t shaded, you can use them to go to any section at any time

Jumps
Use the “Jump to” drop-down list at the top of the page to move to a page you have already completed

WARNING: If you don’t move from one page to another within the report for 30 minutes, your session will expire, and you could lose some of your information.

Other buttons
You may see buttons other than the “Continue” and “Previous” buttons at the bottom of a page. These buttons allow you to delete a page, return to a previous section or take some other action

Pop-up windows
You may see other information in a pop-up window. Use the “Close this Window” button at the bottom of the pop-up window to close it

Enter key
IMPORTANT: Do not use the enter key to move around in the report or to select from the drop-down lists.

 

How can I get a record of what I entered?

  • Once you have completed the report, but before you hit the “Send” button, print a copy of the report or save it for your files.
  • When you hit the “Send” button, the information is transmitted electronically to Social Security.
 

Is this an application for benefits?

No. This is the starting point to apply for disability benefits. Your client must also complete a formal application for benefits.

 

How do we complete a formal application for benefits?

You and your client have three options. You can have your client:

 

When is the i3368PRO available?

You can complete an i3368PRO at your convenience during the hours below (Eastern Time):

Monday – Friday: 5 a.m. until 1 a.m.
Saturday: 5 a.m. until 11 p.m.
Sunday: 8 a.m. until 10 p.m.
Holidays: 5 a.m. until 11 p.m.
