VIII. COMMUNITY AND ORGANIZATIONAL PARTNERSHIP

Goal: Ninety percent of health centers and NHSC DM teams will demonstrate collaboration with State DCPs and at least 75 percent of health centers will utilize National Diabetes Education Program (NDEP) materials and/or initiatives such as Diabetes Today.

Goal: Ninety percent of health center and NHSC teams in each cluster will develop at least one additional community collaboration, such as a partnership with a faith-based organization, community organization, hospital, or school.

Performance Expectations:
PCA and CN

  1. Develop and implement a cluster strategy to facilitate community and State partnerships.
  2. Monitor partnership development utilizing monthly reports and information from agency partners.

In support of this National Goal, CDC plans to:

  1. Partner with PCA and CN to facilitate State partnerships in diabetes care as well as cardiovascular and asthma care.
  2. Share DCP survey information with cluster directors to help strengthen partnerships.

In support of this National Goal, SAMHSA plans to:

  1. Partner with PCA and CN to facilitate State partnerships with health centers and NHSC sites in depression issues.

In support of this National Goal, EPA plans to:

  1. Partner with PCA and CN to facilitate State partnerships with health centers and NHSC sites in environmental health issues.

In support of this National Goal, NCI plans to:

  1. Partner with PCA and CN to facilitate State partnerships with health centers and NHSC sites in cancer issues.

In support of this National Goal, BPHC plans to:

  1. Facilitate partnerships among Federal agencies and BPHC-supported grantees to develop and nourish community partnerships that accelerate the aims of the BPHC-supported collaboratives.
