Destruction of Controlled Substances

!! ATTENTION !!

Failure to adhere to these steps will cause the PDF documents to shrink.  DEA Office of Diversion Control will only accept forms that appear as originals.  The following steps must take place in order to have the PDF forms print correctly:

NOTICE: The instructions provided on the DEA Form 41 are incorrect. Please disregard instruction number 5. That instruction directs the registrant to ship the drugs to the Special Agent in Charge of the DEA office that serves the registrant's area. Registrants should send the forms to DEA as detailed in instruction number 3, and await instructions on how to proceed.

For Acrobat Version 5.0

In the Print Dialog, please ensure the following option IS selected:

1. Auto-rotate and center pages

And, ensure that the following options are NOT selected:

1. Reverse Pages

2. Print as image

3. Print to file

4. Shrink oversized pages to paper size

5. Expand small pages to paper size

In the Print Dialog, please ensure that the following options are NOT selected

1. Reverse Pages

2. Print as image

3. Fit to Page

4. Print to File
