HAVA Reporting

ANNOUNCEMENT: Commissioners recently adopted a policy clarification regarding allowable uses of HAVA Funds. The commission is also seeking comments on Maintenance of Effort (MOE) proposals or about MOE in general. To comment, send an email to HAVAInfo@eac.gov. Please indicate the subject in the email. Go here to view comments. 

HAVA instructs states to report regularly on their use of HAVA funds. To assist in completing reports on funds provided to HAVA, Title I, Sections 101 and 102, and Title II, Section 251, the EAC has prepared separate color-coded model reporting forms (SF 269 - Long Form) for Title I and Title II reports, as well as models of the supporting narrative.

The model forms below show different, acceptable styles of documenting HAVA activities and expenditures during the reporting period, and they are designed to help states meet their reporting obligations. Please note that when using the SF 269 Form, many of the fields auto-calculate. However, the auto-calculation does not always update when making changes. To ensure that your form is calculating correctly, click on the auto-fill field, press zero, and tab to the next field; this will re-calculate the field correctly.
