DEA has Changed Registration
Application Forms
The Drug Enforcement Administration (DEA) is currently phasing in new registration application forms. New scannable forms have been designed that will allow the DEA to process applications more efficiently. The effective date of the new system for processing applications will be: May 1, 1999.
The forms include two additions that will benefit the registrant:
The option of providing a Tax Identification Number and/or Social Security Number. These numbers are used for identification purposes only and will assist in identifying two or more registrants with the same name.
The DEA will also begin accepting credit card payments through Visa ® or MasterCard ®, in addition to checks and money orders, as payment for registration application fees.

Though the appearance of the forms will change, the required information will remain the same. Specific instructions will accompany the new forms.
