Care management services include arrangement for transportation to medical appointments as needed.

Easter Seals NH's Special Transit Service is the largest para-transportation service in the state.

Provides a variety of supports to adults age 60 and older and to adults between the ages of 18 and 60 who have a chronic illness or disability. Services range from home care, transportation assistance and assisted living to nursing home care and legal support services.
