Special Notice

Beginning with the Fourth Quarter 2007 publication, the format for Quarterly Financial Report Summary and Financial tables will change to improve readability for users. The new format will provide table titles and line item descriptions on every page and re-sequence the order of the tables. However, the data line items presented within the tables will not change. The present Third Quarter 2007 publication will be re-issued in the new format during the first week of March 2008.

Internet Reporting

The web-based Quarterly Financial Report Survey is conducted through the Census Bureau's Census taker system.

The Census Taker requires:

  1. A web browser with 128-bit encryption (most browsers released since 1998 are sufficient).
  2. A username and password that is located on the form below the address label.

To complete your survey on-line, please click the following link.

Quarter 2, 2008  (Internet reporting is no longer available for this quarter. Please FAX all reports to 888-353-4102)
