If you are using Acrobat Reader, you CANNOT SAVE the completed form for use at a later time. In order to save completed forms on your PC, you will need to purchase a licensed version of Adobe Acrobat. In order to fill and print these forms using your PC, you must have Adobe Acrobat or Adobe Acrobat Reader version 4.0 or above! If you previously installed Adobe Acrobat Reader 3.0 or installed 3.0 in another package after you installed 4.0, you must uninstall 3.0 to use the forms on this Website without problems. If you have Acrobat configured as a web browser plug-in, you can fill out and print the form within your browser. If you print out a form and it doesn't print completely, print again changing the print settings to "Shrink to fit" or "Fit to page." If you have any trouble printing any of the forms, please consult your organization's Help Desk.
