(EVR) and
In-Station Diagnostics (ISD), it will be necessary to submit an application for a
modification of the gasoline dispensing facility’s existing AQMD Permit to
Operate. A complete application packet needs to include the following
completed application forms:
1. Form
400-A: Application For Permit to Construct and Permit To Operate
2. CEQA Forms: California Environmental Quality Act (CEQA) Applicability
3. Form 400-E-11: Fuel
Dispensing and Storage Equipment
4.
Form 400-CO: Change of Operator
(if applicable)
5. Form 461-CP: Compliance Plan (if
necessary)
In order to avoid delays in the permit processing, please be certain to
complete these forms in their entirety. The permit processing fee is
$1,287.22, if the packet is postmarked no later than June 30, 2009.
The District also offers a voluntary expedited permit processing service.
For those applicants wishing to utilize this service, the permit processing
fee is $1,930.83 (if postmarked no later than June 30, 2009). If expedited
processing is desired, a
Form 400-XPP needs to be included
in the application packet and the accompanying fee.
Construction work should not commence prior to the receipt of an approved permit to

Additionally, a compliance plan is necessary for any gasoline dispensing
facility (GDF) that has not completed a required Phase II EVR upgrade by October 1,
2008. This requirement is explained in details in the FAQs which
accompanies the Compliance Plan Form (Form
461-CP).
