About AD&D Insurance

AD&D insurance protects you and your family from the unforeseen financial hardship of an accident that causes death, dismemberment, or loss of sight, speech, or hearing. The plan provides worldwide coverage for you and your enrolled family members.

Eligibility

You are eligible to enroll in AD&D insurance if your work schedule is at least 43.75% time. You can enroll your legal spouse, domestic partner, and unmarried natural or adopted child, stepchild, legal guardian, or domestic partner's child/grandchild.

You may enroll yourself and any eligible dependents or make any changes to your policy at any time.

Coverage

AD&D insurance offers three coverage options:

  • The Self Plan covers you.
  • The Family Plan covers your spouse or domestic partner and your children.
  • The Modified Family Plan covers you and your children.

LANS rules do not allow dual coverage for AD&D insurance. If you and your spouse work at LANL, you may not cover each other in the plan. You may be covered as an employee or as an eligible dependent of a LANS employee, but not both. You may not be covered in more than one category. If both parents are LANS employees, only one parent can cover the children.

Beneficiaries

All LANS employees must designate at least one beneficiary for their insurance elections. If no beneficiary is named, benefits will be paid to the first survivor in this list (in order):

  1. Employee's legal spouse
  2. Employee's children - in equal shares
  3. Employee's parents - in equal shares, or
  4. Employee's brothers and sisters - in equal shares

If none of these people survives the employee, benefits will be paid to the employee's estate.

2008 Accidental Death & Dismemberment (AD&D) Monthly Payroll Deductions Plan Options

Self Only (employee) Family (employee, spouse/partner, and eligible children) Modified Family (employee and eligible children)

Family Plan Coverage: Covers employee, spouse/domestic partner and eligible children. Your spouse/domestic partner is covered for a Principal Sum equal to 50% of the employee’s Principal Sum and each eligible child is insured for 20% of the employee’s Principal Sum. If you have no eligible children, your spouse/domestic partner’s Principal Sum is 60% of the employee’s.

Modified Family Plan: Covers one parent and all eligible children. If both husband and spouse/domestic partner are eligible employees of LANS, only one may elect to cover their eligible children. Single parents may elect to cover themselves and their eligible children. Under the Modified Family Plan each of your eligible children is insured for an amount equal to 20% of your Principal Sum.

Dismemberment

The plan pays when injury results in certain physical losses (specified in the table below) within 365 days of the date of the accident.

The plan will pay:

100% of principal for
  • loss of two or more members (hand, foot, or eye),
  • loss of speech and loss of hearing in both ears, or
  • quadriplegia.
75% of principal for
  • loss of one member,
  • loss of speech or loss of hearing in both ears,
  • loss of four fingers on the same hand,
  • loss of four toes of the same foot, or
  • hemiplegia.
25% of principal for
  • loss of thumb and index finger of same hand.

Beneficiary Payments

Benefits provided under the AD&D plan are payable in addition to any other insurance you may have when the accident occurs.

If this paperwork is not on file, benefits are paid (in equal shares) to the first survivor in the following order:

  1. your spouse or domestic partner,
  2. your child or children,
  3. your parent(s),
  4. your surviving siblings, or
  5. your executors and administrators,

If you have a beneficiary form on file, it is important that you always keep it current. A will does not supersede a beneficiary designation.
