We are currently accepting single copy (1 per customer) requests for the 2009 Calendar via this form. Duplicate orders and orders from customers who have already placed an order for the 2009 Calendar will not be honored.

Please note: Your order may not ship until December 2008. The shipping date is contingent on printing deadlines, which are subject to change.

Why do I need to supply my phone number?

womenshealth.gov will only use your telephone number to contact you if your Calendar is returned as damaged or otherwise undeliverable.

What is my validation code? What is it for?

The validation code is a random 5-digit number that is generated when you open the order form. You can copy and paste, or re-type the number in the validation code field. We use this to help reduce "form spamming," where automatic form fillers submit bogus requests by repeatedly submitting web site forms. This helps us process your order quicker.

I can't see the form. It says "Thank you for your request ... we can only take one per user."

If you have already submitted a request, you will not be able to submit another one. This form is to request single copies for personal use. If you are an organization requiring multiple copies, please contact our call center at 1-800-994-9662 after September 1, 2008 for assistance.
