This web page will provide you with the information and forms necessary for your appointment to be processed and for you to receive your first paycheck. 

These forms are available for you to complete and bring on your FIRST DAY.   To fill out these forms, click on each of the titles, fill the form out on-line, print and sign it.  Then return the forms to your supervisor or local administrative person.

• Employee Address, AD-349
• Employee's Withholding Allowance Certificate, W-4 
• Employment Eligibility Verification, I-9
• Self-Identification of Medical Disability, SF-256 (as needed)
• Ethnicity and Race Identification, SF-181
• State Tax Withholding Certificate (as needed)
• City and/or County Tax Withholding Certificate (as needed). 
  Your local administrative person will provide you with the appropriate certificate to fill out.
• Statement of Prior Federal Service, SF-144 (if applicable)
• Direct Deposit Sign-Up Form / Waiver Form, SF 1199A
           or
  Fast Start Direct Deposit Form, FMS-2231 (Either form may be used)
• Designation of Beneficiary - Unpaid Compensation of Deceased Civilian Employee, SF-1152
