The Public Health Service Officers Device Supply Center is pleased to share with you our web site. Award plaques are also available with Public Health Service or Commissioned Officer insignias. The Public Health Service Officers Device Supply Center is designated in the Commissioned Corps Personnel Manual (CCPM), Subchapter CC27.1, as the official source for replacement of Public Health Service award items. Programs and officers may order by two methods. We offer our online ordering program, through Pay.gov, which accepts VISA, MasterCard or Discover payments, or you can mail the interactive form with check payment. We offer express mailing through Federal Express Delivery or standard U.S. Postal Service.
