If you need immediate proof you applied for a Social Security card, we can issue a receipt. Simply ask for one when you go into your local Social Security office with identification documents to apply for a card. If you mail your materials, include a written request for an application receipt. We will issue an application receipt only when your application is complete, all your identifying documents are in order and we deem the application ready for processing.
To apply for a Social Security number and card:
- Complete an Application For A Social Security Card (Form SS-5); and
- Show us original documents or copies certified by the issuing agency proving:

- U.S. citizenship or immigration status [including Department of Homeland Security permission to work in the United States];
- Age; and
- Identity.
In most cases, you can mail or take your application and original documents to your local Social Security office. If you live in the New York City metropolitan area, Las Vegas NV, Orlando FL, or Phoenix AZ you may need to apply in person at your local Social Security Card Center. First time applicants age 12 or older also must apply in person.
