OF REAL PROPERTY VALUATION DETERMINATION

of two steps: Filing this petition and submitting evidence to
support your appeal. This petition must be filed or postmarked
within 60 days after the date of mailing of the Assessment Notice,
Change of Value Notice, or other determination notice.
A copy of the most recent
assessed value notice must be attached to this petition form.
Petitioners MUST print the form, sign and file it. Electronic
filing is not allowed by law.
