Designate Power of Attorney

Why

At some time, you may want or need someone to deal with PBGC on your behalf. You can do this with a power of attorney form. This form allows you to authorize another person to act as your agent to do specific things, such as apply for pension benefits or respond to PBGC’s request for information or documents.

Note: You cannot use this form to designate a power of attorney that applies during incapacity (a durable power of attorney). If you have questions about a durable power of attorney please call our Customer Contact Center.

How

    • Remember to have your social security number, plan name and case number ready for the customer service representative so that they are better able to help you.
