Project Reporting Toolkit
The following forms and links are intended to help recipients of NN/LM PNR outreach funding with their reporting obligations.
Outreach and consumer health subcontractors in the Pacific Northwest Region should submit Quarterly Reports within 10 days after the end of each quarter and a Final Report at the completion of the project.
Subcontractors and mini-award recipients should use the appropriate forms for training sessions (Outreach Activity Data Collection Form and Participant Information Sheet) or exhibits (Exhibit Report and Budget Forms) conducted during the quarter. Completed forms should be mailed, faxed, or emailed to the NN/LM PNR. Transmittal information is given below.
