Claiming FEGLI Life Insurance Benefits



 

To claim FEGLI benefits for the death of an employee or annuitant:

FE-6 -- Claim for Death Benefits

To claim FEGLI benefits for the death of a family member of an employee or annuitant:

FE-6 DEP -- Statement of Claim, Option C (NOTE: This electronic form requires Adobe Acrobat 5.0 or above.)

These forms are screen-fillable Adobe Acrobat PDF files. Using the free downloadable Adobe Acrobat reader you may complete the form on the screen and sign it. Note, however, that if you are using the free Acrobat reader software you cannot save the form with the information that you have typed in it on the screen. You need the commercial Adobe Acrobat program in order to save the form with the data. More information is available on our Adobe Acrobat forms help screen.

Be sure to read the instructions on the form. You will have to print out the claim form. It cannot be submitted on-line.

If you or someone else hasn't done so already, you also need to report the death.

The address for submitting the FE-6 claim form is:

The address to use for overnight mail deliveries only (such as Fed Ex) is: 2 Montgomery Street, Jersey City, NJ 07302-3802.

The FE6-DEP form is submitted to the agency employing office for employees and the OPM Retirement Operations Center for annuitants.
