  When must I file my request for review of the Administrative Law Judge's decision?
  Answer
  If you wish to appeal, you must make your request in writing within 60 days from the date you received the hearing decision or dismissal. We assume you received the decision or dismissal five days after the date on it, unless you can show us you received it later. If you have a good reason for requesting review after the 60th day has passed, you must explain that in your request.
