Form SS-5, Application for a Social Security Card, is the appropriate form to change citizenship information on our records.

To change the the citizenship shown on our records:

  • Your new or revised citizenship status;
  • Age; and
  • Identity.

All documents must be either originals or copies certified by the issuing agency.  We cannot accept photocopies or notarized copies of documents.

It may take a few weeks for us to verify your information and change your record. If needed, we can provide you with a letter acknowledging you requested the change.
