Coast Guard ICS Forms

Incident Command System (ICS) forms are one of several tools that are available to support personnel managing a response to an incident and in completing an Incident Action Plan (IAP). There are several different "versions" of the ICS forms:

  • The original National Fire Equipment System ("NFES") forms, which are wildfire specific and were copied into the National Incident Management System (NIMS) guidance. Several of these forms are not set up for all-risks/all-hazards.

  • Coast Guard and NOAA oil spill specific ("OS") forms.

  • Coast Guard ("CG") forms that have been developed for all-risk/all-hazard situations.

Some of the forms are almost identical between the various versions, and a few have not been adapted from the NFES versions at all. The Coast Guard generally recommends the use of the CG ICS forms because they are the most capable for all-risk/all-hazard incidents; however, any of the three versions may be used.
