WHAT INFORMATION SHOULD YOU PROVIDE WHEN CONTACTING THE IG?

In order for the IG to act on your complaint or requested assistance, you should try to provide as much of the following information as possible:

  • State the nature of the problem (who, what, when, where, how, and why)
  • Identify any documents, personal notes, memoranda's of record, etc. which will support you complaint
  • Provide the names of individuals/entities involved and other persons who know about the problem
  • You can remain anonymous. However, if you want us to respond back, you need to identify yourself to the IG
