You can request a replacement Medicare card online. To make your online request, you will need:

  • Your name as it appears on your most recent Social Security card
  • Your Social Security Number
  • Your date of birth

The address for this service is http://www.socialsecurity.gov/medicarecard/  Your Medicare card will arrive in the mail in about 30 days.

If you prefer, or if you are unable to use the online request to obtain a replacement Medicare card, call our toll-free number, 1-800-772-1213. Our representatives there will be glad to help you.
