The Outreach Sourcebook (Volume 5, Rural Health Demonstration Projects, 1995-1998)

This report provides a summary of 25 outreach demonstration projects awarded by HRSA's Office of Rural Health Policy in 20 States. It gives an overview of individual grantees' experiences, what worked, what did not work, and how they reevaluated their goals and objectives to meet the needs of specific communities. The report contains a map of the project locations and contact information for each project.
