Abandoned Vessel Reporting Form

Use this form to submit more information about a vessel currently in
the inventory or a new vessel you don't see listed. Thank you.

Vessel Name:

Vessel Location: Geographic coordinates are helpful. If you are relating the vessel to landmarks, please roughly estimate distances and orientation (ie. The vessel is about 100 meters SW of the first red buoy when entering the harbor.)

Location Description: What is the substrate under the vessel? Is the vessel submerged? What kinds of natural habitats are nearby? (ie. The vessel is resting on sand, partially submerged in 10 feet of water with hard coral 100 feet to the west.)

Vessel Description: Is the vessel whole or in pieces? What size and type is it? What is the hull made of? How long has it been abandoned? (ie. Only about 50 feet of the bow remains. It is steel barge and has been there for at least 10 years.)

Pertinent Legal Information: What is the vessel's flag state?
Is a registry number visible? (ie. The vessel is American, #AA1234AA.)
