Case NumberCase numbers must be provided on recommendations for waiver of the two-year foreign residence requirement that HHS forwards to the U.S. Department of State. Any cases now pending before HHS will be required to have this case number before an approval can be forwarded. Applicants to HHS have the option of obtaining the case number before submitting the application, or waiting to obtain the case number after HHS makes its decision. It may take up to eight weeks to obtain a case number. U.S. Department of State Procedures Applicant organizations can obtain a case number by following the Department of State instructions below. - Complete Form DS-3035: J Visa Waiver Recommendation Application (also known as the Data Sheet).
- Send the completed Data Sheet; two self-addressed, stamped, legal-size envelopes; and a cashier’s check (personal checks are not accepted) or money order for $215 per application, payable to the U.S. Department of State.
- Write on the cashier’s check or money order the following applicant information:
- Waiver case number, if known
- Name
- Date and place of birth
- Social security number
