FY 2006 Reporting Instructions for the Federal Information Security Management Act and Agency Privacy Management (M-06-20), July 17, 2006. The instructions require:

That agencies include an attachment or appendix with the results of the privacy review the agency's senior official for privacy conducted according to OMB Memorandum M-06-15 of May 15, 2006, Safeguarding Personally Identifiable Information;
That agency Inspectors General provide a list of any systems they have found missing from the agency's inventory of major information systems;
That agency privacy updates be submitted quarterly with security updates to support the President's Management Agenda and
That agencies identify any physical or electronic incidents involving the loss of or unauthorized access to personally identifiable information and report them in accordance with OMB Memorandum M-06-19 of July 12, 2006, Reporting Incidents Involving Personally Identifiable Information and Incorporating the Cost for Security in Agency Information Technology Investments.
