SE Annual Agreement Form Guidelines


Scientists Emeriti must complete an annual on-line USGS Individual Volunteer Services Agreement Form 9-2080 by October 15 each year.

Ethics: Scientists Emeriti are expected to follow ethics rules applicable to their status and position. Written agreement to abide by the Ethics Rules for Scientists Emeriti, http://geology.usgs.gov/Emeritus/about/policies/ethics.shtml, is an expected "condition of acceptance" into the Emeritus Program and continued adherence to them is a “condition of continuation” in the Emeritus Program.

How to find Form 9-2080

  • The on-line Individual Volunteer Services Agreement form, https://gsvaresa08.er.usgs.gov/WebForms/9-2080.nsf, is now easily accessible using your Lotus notes ID and internet password. If you don't know your internet password, you can either call the Help Desk at 703-648-4357 or email them at GS Help Desk (from inside the USGS) or GS_Help_Desk@usgs.gov (for those of you outside the USGS facilities).

Instructions for Page 1

  • Section 3 – If the calendar does not pop up when you click on the icon, please minimize the form and you will see the calendar in small window.
  • Ignore the signature section.

Instructions for Page 2. Go to page 2 by selecting “Page 2” on top left portion of page 1.

  • Section 4:
    • Honorary Scientists Emeriti do NOT have to complete section 4. Honorary Scientists Emeriti are individuals who have had long, distinguished careers with the USGS and who are now less involved in USGS science activities. These SE's are assigned to an organizational unit based upon their geographic location and receive some limited resources, such as access to a phone, email, and the internet.
    • All other Scientists Emeriti must include the following items in section 4a (do not use section 4b). If you need more than 10 lines, select “Cont. Page” from top left of page.
      1. Address (including room number) where you conduct your Scientist Emeritus activities.
      2. Preferred e-mail for receiving Scientist Emeritus information.
      3. Duties/work that you plan to accomplish during the fiscal year. This may be identical to your Bradley proposal.
      4. All USGS projects with which you will be participating in the fiscal year, including Project Chief name.
      5. Indicate if you will be applying for the Bradley Scholar Program.
      6. List your funding needs for the fiscal year that are NOT included in a Bradley proposal. Include how the money will be used: publication costs, purchase of maps, field work, laboratory expenses, attend meetings, drafting, GIS , secretarial, etc.
      7. Discuss what you accomplished during the previous fiscal year. This information MUST be provided annually after you have completed your first year as a Scientist Emeritus. Give a complete reference for all products published in the past year. List all other products, and indicate whether they are in progress, in review, Director’s Approval, etc. This information will be placed on the Geology Scientist Emeritus website and incorporated in the Annual Geology Scientist Emeritus report.
    • Select USGS Supervisor. Click on “Select USGS Supervisor.”  Enter your Team Chief Scientist’s last name. Click the “Find” button. Click on the correct name and it will appear in the box on the right. Click the “Assign Name” button. This will automatically enter all the additional information for the person. If it doesn't’t, don’t worry about it.
  • Section 5: Select USGS Official. Use the same process as above. However, enter the name of your Regional Executive.
  • Section 6: Each SE must maintain a detailed time sheet to ensure coverage in case of injury.
  • Section 7: Additional Information. Check either c, Scientist Emeritus, or d, Scientist Emeritus Honorary.
  • Section 8: Hours Worked and Expenses Incurred. Under fiscal year, enter current year and then enter hours worked and expenses incurred for each quarter as it is completed.
  •  Bottom of page: List of names of individuals who may access this form for reporting purposes: Please select (1) your Team Chief Scientist, (2) your Regional Executive, and (3) Judy Back, Geology Scientist Emeritus Coordinator.
  • PRESS THE “SAVE” BUTTON!!!!!
  • Print out and review the agreement form with your Team Chief Scientist.
  • You and your Team Chief Scientist should sign a copy of the final agreement.
  • The original copy is kept in the Team office.

Updating/Making corrections after you complete and save Form 9-2080

  • Enter the Form 9-2080 site: https://gsvaresa08.er.usgs.gov/WebForms/9-2080.nsf
  • Click “Return to Main Menu” at the top or bottom of the page.
  • Click the triangle to the left of “U.S. Geological Survey Forms (9).”
  • Find Form 9-2080 (on the bottom of second page).
  • Select “View/Print”.
  • Clicking on any underlined heading will get you to your previously filled out information.
  • Make any corrections and “Save.”
