Congressman Pitts can help you with this matter.  First, the law requires that you give him written permission.  Please print out a Privacy Act consent form, fill it out and sign it, and mail it to Congressman Pitts' office for the county you live in.  You may want to attach copies of any pertinent paperwork.  After a few days, call the office you mailed the form to and ask for a staff member to assist you.  Keep in mind that federal agencies won't share any information with the Congressman or his staff until we have the consent form.  If you need help filling out your form, feel free to call right away.  You'll need Adobe Acrobat Viewer to download the consent form.  It can be downloaded for free.  We can also mail you a form.
