Passport Needs

My office is pleased to assist constituents with your passport needs.  For those who require passports for the first time or need to replace or renew passports, the information below should prove useful.  Passports are typically processed in four weeks.  If you are requesting a departure date of less than 14 working days from the time of the application and need your application expedited, the Passport Service requires a fee of $60 in addition to the regular fees for the passport.  It is strongly suggested that applicants requiring expedited handling also request overnight mail delivery service to and from the Passport Agency in addition to the expedited service, for an additional charge.  If you must travel overseas in less time than permitted by normal channels or require additional assistance, please do not hesitate to contact my El Dorado office at (870) 881-0681 for advice.

New or First Time Passports:
New or First Time Passports require a completed DSP-11 form, $75 application fee (check or money order made payable to Passport Services), a $25 execution fee (made payable to the facility where you are applying), two identical passport photos (please read the application instructions regarding required size), and proof of citizenship (a certified copy of the birth certificate, naturalization document or expired passport).  If a certified copy of a birth certificate is not available, the passport agency requires a letter from vital statistics stating there is no record of the birth and as many of the following pieces of information: affidavit of birth, baptismal certificate, hospital certificate, early school records, or census reports.  Applications for new passports will not be processed unless signed in front of an authorized accepting agent and sealed into a brown envelope.

Lost or Stolen Passports:
Lost or Stolen Passports require the applicant to follow the directions for a new passport and include a DSP-64 form.

Passport Renewal Applications:
Passport Renewal Applications require a completed DSP-82 form, $75 application fee (check or money order made payable to Passport Services), two identical passport photos, and the expired passport.  In order to apply for a renewal, the applicant must have been 18 years old when the passport was issued and the expired passport must have been issued within the last 15 years.  These applications should be mailed to the National Passport Center, P.O. Box 371971, Pittsburgh, PA 15250-7971.  If expedited processing is requested, an expedite fee of $60 should be added to the application fee.
