Constituents frequently ask questions regarding Social Security, Medicare, the Internal Revenue Service, Citizenship and Immigration Services or other federal agencies. If you have a problem or question concerning an agency, the District Staff is prepared to assist you.
Under the Privacy Act of 1974, federal privacy laws prohibit any inquiries on your behalf without your written permission. In order for our staff to begin work on your request, individuals must complete a Privacy Release Form located below and return it to a district office, along with any other documents that may help resolve their case (Please send only copies of original documents).
