Please contact us if you are having a problem with any federal agency. While we cannot instruct a federal agency to take any particular action, we can make sure that your matter receives expeditious attention. Where necessary, we can make sure that your matter is carefully reviewed by a supervisor.

We are required to receive this authorization before we may assist you. Upon completion of this form, please mail with any supporting documentation to the address at the bottom of the form.

Our office recommends a select group of high school graduates for admission to the U.S. Military Academies, we arrange tours of the White House, the Capitol and other sites when our constituents visit Washington, D.C., and we can assist with passport applications and answer questions about visas.
