All of the forms on this page may be printed and submitted in person at the Utility Billing service window located on the second floor of Town Hall.  Customers may also opt to submit them electronically as outlined under For electronic submissions below.   Please be sure to read all instructions printed on the forms and on this page.
Account Changes or Disconnect Service   

This form is used to close your account, change your mailing address or change your name. Legal documentation is required.         

Bank Draft Application 
This form is used to authorize the City to set up automatic bank draft to pay your monthly bill.  Please print the form after completion and submit to us in person or by fax or e-mail.  We are working on an automated online submission for this form as well.
Bill Adjustment Request  

This form may be used for either a leak adjustment or a penalty waiver.  Leak adjustments must be accompanied by a paid invoice from a licensed plumber.
Please note, penalty waivers are granted by the City only once per year.

Utility Service Application
 
For in person submissions:
  • bring a deed of ownership or a lease agreement for residential accounts or
  • bring legal business documents for non-residential accounts
          
For electronic submissions:

           
All applications must include a deed of ownership or lease agreement for residential accounts or legal business documents for non-residential accounts.  Missing information or documents may delay connection.


A UB staff member will contact you to let you know the application was received and the account is established.
