VA Pittsburgh Healthcare System Director Terry Gerigk Wolf was formally removed Thursday from her position by the U.S. Department of Veterans Affairs.

Wolf, who has been on administrative leave since June, was fired for conduct unbecoming of a senior executive and wasteful spending, claims that were substantiated after an investigation by the Office of Accountability Review. Dr. David Macpherson has been acting director since June and the VA has begun the search for a new director.

"This removal action underscores VA's commitment to hold leaders accountable and get veterans the care they need," the VA said in a prepared statement. Wolf was not available for comment. The VA recommended her dismissal in October.

Wolf was criticized for her handling of an outbreak of Legionnaire's disease in 2011 and 2012. Six veterans died as a result of the outbreak, which also sickened others.
