Eligibility Requirements
This program honors champions of health in North Texas.
- The website to officially apply is currently under construction. In the meantime, please submit your contact information on this nomination form, and we will make sure you receive all necessary information to apply for this award program once it is available.
Eligibility criteria:
- Be a business or not-for-profit entity in North Texas.
- Employ a minimum of five full-time employees, including the owner.
- Demonstrate evidence of an employee wellness program.
- Complete the online application (once available).
- To be eligible for this program, your company must have a minimum of five employees in the seven-county Dallas-Fort Worth metropolitan area: Dallas, Collin, Denton, Tarrant, Rockwall, Ellis and Kaufman counties.
- Companies being honored are required to attend the awards luncheon and health fair on March, 19, 2015 from 10 a.m. - 2 p.m.
