Guidelines for Filing Computer-Generated Hardcopy Forms

Information on gaining approval to use computer-generated form templates created outside of the RRC (by operators, consultants, or software vendors) before submitting such forms to the RRC.

Get approval from the RRC Oil and Gas Division for each series of forms you wish to file (for example: the H-15 is a series, etc.). If a software vendor has already received Commission approval for a series and it has been assigned a unique identifier (see no. 4 below), individual filers using that series software do not need to seek further approval as long as the vendor's approval identifier appears on each form filed and the form and print characteristics of the data do not vary from what was originally approved. Changes in software or equipment such as printers after a form has been approved may make it unacceptable. Vendors are to advise software users of these requirements as well as the Form Standards (see box to right below)

FORM STANDARDS

Forms may be revised and standards may be changed at any time. Approved form series may have to be updated and resubmitted when this occurs.

  • Appearance of the form series template must closely resemble the actual hard copy Commission-printed form, not the website format. When the Commission develops scanning capability for data update, the template must be an exact match. At this time, the following must be an exact match: H-5
  • Use comparable font style and size. Use the same vertical and horizontal spacing, paper size, and margins (the top margin must be no less than 0.5").
  • All data fields must be included and of the same size as on Commission-printed form, even if there is no data in them. · Use black ink and, except for the following, white paper: P-1B (pink paper).
  • Do not shade any areas even if they are shaded on the Commission-printed form (except for the P-5 form).
  • To fill in data, use 10 (Arial only), 11, or 12 point size. The inking must be black and of good, dark quality, not faded. All characters must be fully formed.
  • Instructions do not have to be printed on the back of forms with the following exceptions: H-1 (with certification section), H-7, H-10H, H-11, P-7, P-13, W-3A, W-14, P-1
  • The certification section of the H-1 must be printed on its reverse side
  1. Submit (by mail, NOT fax) two forms created by the software: one blank and one partially filled in with "z's" and "9's" in data fields. Clearly mark both "SAMPLE". Send to: FORMS COORDINATOR OIL AND GAS RAILROAD COMMISSION OF TEXAS P. O. BOX 12967 AUSTIN, TEXAS 78711-2967
  2. Include the following information: name of your company, its address, the name and phone number of a contact, and what program, if any, you used to create the forms software (i.e, Excel, WordPerfect, QuattroPro, Lotus, etc.).
  3. You will be contacted when the form is approved for use or if there are any problems with it.
  4. Approval Identification. With the approval of your forms series, you will be assigned a unique approval identifier that must appear on all computer-generated forms from that series that are filed with the Commission. The identifier may appear at either the top or bottom of theform, preferably at the top right, under the form designation and revision/creation date.
